& submit form to us & proceed to next step

Artist Image is 100 pixels wide X 150 pixels high at 72 dpi
Artwork Image pictures: 150X150 pixels at 72 dpi - finished OR 150 pixels on widest side of image-
we will put the image in a "box" for you

We will email you with further instructions.
Be sure to list the FILENAME in the spaces provided so we will be able to identify your
files when we receive your email with the images.
